When needed, we help you and your family manage the transition from home health to a higher level of hospice care seamlessly. We also do our best to keep the same nursing staff in place, further easing the transition for you or your loved one.

Eligibility Requirements

Who is eligible for Bluebonnet’s Bridge Program?

Those suffering from life-limiting illnesses, including but not limited to:

  • Cardiovascular Disease
  • Congestive Heart Failure
  • Cancer
  • Stroke or Coma
  • Pulmonary Disease
  • COPD
  • Liver/Renal (Kidney) Disease
  • Multiple Sclerosis
  • ALS/Lou Gehrig’s Disease
  • Alzheimer’s Disease
  • Parkinson’s Disease
  • HIV Aids

 

Regardless of a patient’s diagnosis, the following may also mean you are eligible for Hospice care:

  • Uncontrolled or increased pain
  • Declining mental abilities
  • Recurrent infections
  • Frequent hospitalizations
  • Recent decline in functional status
  • Increased breathing difficulties
  • Increased weight loss

Medicare and Medicaid will cover 100% of the cost of our services. We also accept most private insurance and will verify coverage with your insurance carrier to determine if any co-payment is required.
